Program Specialist
1. Develop and implement a plan of care appropriate for each individual to ensure residential program services are delivered as written
a. Direct and participate in the development of Individualized Supports Plans (ISP) and Person-Centered Supports Plans (PCSP) that meet the needs of individuals
b. Coordinate the timely completion of assessments and other documents required for plan of care creation, review, and revision.
c. Schedule and invite appropriate attendees for plan of care meetings, and document the meeting
d. Prepare and send documentation as required
e. Write monthly service notes based on review of staff documentation
f. Write progressive goals and monitor progress on accomplishing goals
g. Document quarterly review of individual goal attainment, medical and other updates, and review with the individual
h. Provide liaison to the Supports Coordinator or Service Coordinator
i. Communicate changes to the plan of care as appropriate
j. Communicate changes in health or behavior as appropriate
k. Communicate regularly with family members to exchange information in support of the individual
2. Coordinate with the Residential Homes Manager(s) to support human resources activities to provide appropriate and trained staff in each home.
a. Assist with on-boarding activities for new hires, as requested
b. Provide Direct Support Professionals (DSP) training on individuals’ plan of care including health, safety, goals, desired activities, and preferences
c. Provide medication administration training and observations, and oversee observations conducted by others
d. Provide support and modeling to residential staff on plan of care goals and encouragement to promote individual choice
3. Oversee residential services to ensure the health, safety, security, and community participation of individuals with the following activities:
a. Monitor and support the provision of social and other activities according to the plan of care
b. Arrange and maintain individuals’ ACCESS enrollment for transportation
c. Coordinate, procure, and provide training on special equipment
d. Monitor required assessments and appointments and support staff in their timely completion
e. Update individuals’ informational forms, as needed
f. Update individuals’ medication lists and communicate with the pharmacy
g. Audit individuals’ records for timeliness, accuracy, and completeness in compliance with applicable regulations
h. Communicate information related to discharges from the home.
i. Participate in the process of admission of new individuals to the home.
4. Complete required documentation and data collection to ensure compliance and quality
a. Timely file information in individuals’ records in compliance with regulations and agency policy
b. Report critical incidents and function as point person to process reported incidents via the EIM system
c. Work with Quality Compliance and Privacy Coordinator to ensure compliance with relevant funding source regulations and CLASS quality management plan including monthly record audits and direct observation.
5. Perform duties in a manner that promotes and supports the mission, vision, and values of CLASS to ensure effective and efficient services are provided
a. Support individual choice, the achievement of desired community integration, and the person-centered philosophy
b. Communicate in a professional, discreet, and tactful manner
c. Complete tasks in a self-directed, organized, and efficient manner
d. Maintain positive working relationships with co-workers and supervisors.
e. Render and accept criticism in a professional manner
6. Perform other duties, as assigned.
MINIMUM QUALIFICATIONS:
1. A master’s degree or above from an accredited college or university and 1 year of work experience working directly with individuals with an intellectual disability or autism.
2. A bachelor’s degree from an accredited college or university and 2 years of work experience working directly with individuals with an intellectual disability or autism.
3. An associate’s degree or 60 credit hours from an accredited college or university and 4 years of work experience working directly with individuals with an intellectual disability or autism.
4. Working knowledge of applicable regulations and requirements
5. Current Criminal History Clearance

NON-ESSENTIAL FUNCTIONS:

Physical Demands/Requirements
1. Ability to see and interact with text and graphical information on a screen.
2. Ability to receive and respond to phone calls.
3. Ability to use office equipment.
4. Ability to lift 50 pounds.
Description of Work Environment/Conditions
1. Occasionally travel locally with the individual.
2. Regularly travel to residential homes locations.
3. Regularly work non-traditional hours including participation in on-call rotation for incident management reporting duties during nights, weekends, and holidays, as assigned
4. Occasionally fill in for a direct support professional shift including during non-traditional hours, as needed.
5. Regularly work in a climate-controlled home or office environment or another approved location in the community.
