TITLE: Direct Care Worker - Part-time position

BASIC FUNCTION: To provide adults with disabilities and senior citizens with In-Home Services on a regularly scheduled basis.

ESSENTIAL FUNCTIONS:
  • Provide non-medical in-home services to eligible adults in their homes to ensure their needs are met according to the plan of care.
  • Provide one-on-one in-home ADL (Activities of Daily Living) that is outlined in the participant’s plan of care.
    • Duties may include, but are not limited to assistance with: transfers, bathing, toileting, dressing, and grooming.
  • Provide one-on-one in-home IADL (Instrumental Activities of Daily Living) that is outlined in the participant’s plan of care.
    • Duties may include, but are not limited to assistance with: meal preparation, shopping, arranging transportation, facilitating appointments and meetings, and assistance with communication needs.
  • Provide housekeeping services, as directed.
    • Duties may include, but are not limited to: laundry, changing linens, vacuuming, dusting, washing dishes, and mopping floors.
  • Perform health maintenance activities as stated in participant’s plan of care.
  • Consistently use EVV (Electronic Visit Verification) system to ensure complete and accurate information for billing of services provided.
  • Review EVV for scheduled shifts and required tasks
  • Clock in and out of every scheduled shift
  • Accurately identify and enter tasks completed for the participant
  • Promptly notify supervisor of any EVV problems
  • Consistently follow all CLASS and In-Home Services policies to ensure the health and safety of the participant.
  • Promptly report suspected abuse or neglect to supervisor.
  • Promptly report participant falls, injuries, 911 calls, Emergency Room visits, and hospitalizations to supervisor.
  • Promptly report participant physical and behavioral health changes and other concerns to supervisor.
  • Participate in CLASS Quality Management Plan, as required.
  • Comply with CLASS planned and unplanned absence policy to ensure participants receive services according to their plan of care.
  • Promptly notify supervisor of requests off or schedule changes needed.
  • Provide 6 hours advance notice when calling off for scheduled shifts.
  • Notify supervisor of inability to attend scheduled training.
  • Perform other duties, as assigned.

MINIMUM QUALIFICATIONS:
  • Must have a high school diploma or equivalent.
  • Experience working directly with individuals with disabilities.
  • Must be a minimum of 18 years old.

REQUIRED SKILLS:                          
  • Positive and effective communication skills.
  • Basic reading, writing, and math skills.
  • Ability to treat participants with respect and dignity.
  • Basic cooking and housekeeping skills.
  • Ability to follow a participant’s plan of care.

OTHER JOB REQUIREMENTS:
  • Have access to reliable transportation to travel to the participant’s home.
  • Have access to a mobile phone and internet service.
  • Perform duties in a professional, mature, effective, discreet and efficient manner.
  • Be a self-directed, organized and responsible professional.
  • Respond to staff and participants in a tactful and professional manner.
  • Adhere to the policies established by CLASS and its relevant funding sources and Board of Directors.
  • Maintain positive working relationships with program director and co-workers.
  • Render and accept criticism in a professional manner.
  • Attend trainings related to job functions as required.
  • Check and respond in detail to CLASS emails in a timely manner.

WORK ENVIRONMENT & PHYSICAL DEMANDS/REQUIREMENTS:
  • Ability to see and interact with text and graphical information on a screen.
  • Ability to receive and respond to phone calls.
  • Regularly travel locally to the home of a participant.
  • Regularly work non-traditional hours including nights, weekends, and holidays.
  • Regularly work in a climate-controlled office environment or in the home of a participant.
  • Ability to lift 50 pounds.
