REPORT: Supervision received: Senior Residential Homes Manager
                  Supervision exercised: Lead Direct Support Professionals (DSPs) and DSPs


ESSENTIAL FUNCTIONS:

  • Work with Human Resources Recruiter in the interviewing and hiring of new staff for assigned home as needed.
  • Conduct on-site training, with corresponding documentation, with newly hired staff.
  • Complete performance evaluations on new staff working within 90 days of employment and annually thereafter using individuals’ input as well as firsthand observations of employee’s job skills and performance.
  • Complete monthly house schedule.
  • Oversee and approve assigned staffs’ timesheets, including approving and tracking PTO.
  • Fill in vacant direct care shifts in emergency situations or staff shortages.
  • Carry out disciplinary actions per agency guidelines under the supervision of the appropriate program director or designee.
  • Conduct staff meetings on a regular basis (at least monthly).
  • Serve as on-call support for assigned home via agency provided cellular phone.
  • Participate in department on call rotation as scheduled per program needs.
  • Ensure that physical site meets regulatory compliance standards including submitting maintenance repair requests, creating menu, completing grocery order, etc.
  • Maintain site vehicle and schedule maintenance as required.
  • Provide medication oversight for assigned individuals including completing weekly medication and MAR reviews.
  • Update individuals’ medication lists and communicate with the pharmacy for any medication needs including, but not limited to, individual copays, prescription changes, refill requests, etc.
  • Review assigned individuals’ personal financial accounts on at least a weekly basis, ensuring that all bills in the individuals’ name are paid in a timely manner. Reconcile financial ledgers with individuals’ bank statements on a monthly basis.
  • Attend assigned individuals’ annual ISP and monitoring meetings and collaborate with team in plan creation and implementation.
  • Schedule and accompany assigned individuals to healthcare appointments, ensure that immunizations, written recommendations of physicians and therapists and all medical follow-ups are completed as directed.
  • Coordinate community activities with/for individuals and ensure appropriate staff support is in place.
  • Assist individuals with arrangements for transportation via agency van and/or ACCESS, monitoring ACCESS account balance weekly and requesting funds as needed.
  • Complete documentation as required. i.e. Census, Service Notes, MAR documentation, etc.
  • Ensure assigned staff complete required documentation daily.
  • Communicate with Lead DSP on a daily basis.
  • Provide team communication updates for each individual at intervals as requested, but no less than monthly.
  • Provide guidance and support to staff and individuals in assigned home.
  • Work with Quality Improvement and Compliance Coordinator to ensure compliance with relevant funding source regulations.
  • Report critical incidents immediately, but no later than 24 hours, following the knowledge of a reportable incident and provide all necessary information.
  • Complete ODP Medication Administration Practicum Observer training within six months of hire and complete assigned staff’s annual requirements thereafter.
  • Monitor staff training hours and ensure completion of trainings as required.
  • Complete at least 24 hours of assigned training annually.
  • Other duties as assigned.

MINIMUM QUALIFICATION:
  • Associate’s degree or equivalent schooling in a related field and at least two years residential as well as supervisory experience.
OR
  • High school diploma and five years of residential experience and demonstration of leadership abilities.

  • Organized and detailed, excellent in multi-tasking, and able to handle extensive amounts of paperwork/documentation.

  • Excellent written and oral communication skills, as well as exceptional interpersonal communication skills.
  • Possession of strong analytical skills and sound judgement.
  • Self-directing and independent with ability to work and make timely decision with little direct supervision.

WORK ENVIRONMENT & PHYSICAL DEMANDS/REQUIREMENTS:
  • When work is performed in an office setting and sedentary; requires sitting for long periods of time.
  • Requires full range of body motion including manual finger dexterity and eye-hand coordination for use of calculator and computer keyboard.
  • Sitting, standing, walking, typing, turning in chair, stooping and reaching for files.
  • Ability to receive and respond to phone calls; face to face conversation.
  • Ability to see and interact with text and graphical information on screens, read forms and printouts.
  • Ability to use office equipment such as copiers, scanners, computers, telephones, and fax machines.
  • Travel to assigned Residential home sites, transport participants to activities and appointments.
  • Regularly work in a climate-controlled office environment or another approved location
  • Ability to lift 30 pounds.
